Exsanguinous is an extremely rare item with no concrete way to target farm. That being said, there are a couple of ways through which you can make the process of farming it a bit easier. The best areas to target farm Exsanguinous in Last Epoch are Empowered Monoliths and Blood, Frost, and Death.
Try Farming For Exsanguinous In Zones Closer To Level 23
At higher levels, you might ‘pollute’ the drop pool by finding unique items with higher levels. As Exsanguinous becomes ready to be availed at level 23, your best shot is hanging around zones in that level. You can also try target farming Exsanguinous during the Blood, Frost, and Death timeline in Monoliths of Fate.
Demolish everything
Exsanguinous is not a specific drop. You can simply farm it by slaying any enemy. You do not need to focus on certain areas or types of enemies. It is a non-gambleable unique with extremely high rarity. So your best shot is slaughtering incoming mobs of enemies in areas and timelines like Omen of Silence.
