To craft and get the secret Rune Pistol, obtain the Ravenous Medallion from The Great Hall in the Losomn realm and offer it to the strange blue webs in The Tormanted Asylum prison cell. This will give you a Decrepit Rune in return which can be used to craft the Rune Pistol.

Keep in mind that due to the procedurally generative nature of Remnant 2, you might not get these locations in your playthrough of the Losomn realm. If that is the case, complete the whole realm at least once, then re-roll the realm till you get Morrow Parish as your starting location, This means you got the Nightweaver storyline and you will get access to The Tormented Asylum.

The Rune pistol is a powerful Hand Gun that is most effective at medium to close range. It also has a unique weapon mod that comes pre-equipped and cannot be removed. How to get Ravenous Medallion for the Rune Pistol in Remnant 2

Below is a step-by-step guide on how to find and acquire the Ravenous Medallion and how to use that to unlock Rune Pistol:

1. Make sure you are in the Nightweaver storyline for Losomn by re-rolling until you get Morrow Parish as your starting location.
2. Progress through the realm and find The Great Hall location, The entrance to this location usually spawns in the Forgotten Quarter location but it can potentially be different for you.

   

3. In The Great Hall, you will find an area that looks like a kitchen, Explore this area and you will find the Ravenous Medallion ready to be picked up with a red glow around it.

   

4. Once you have the Medallion, continue the Nightweaver quest line until you get the Soulkey Tribute, This cannot be missed and is obtained by simply progressing through the main quest of the realm.
5. Using the Soul Key tribute on the mysterious blue thing covered in webs at the basement of the Asylum (Morrow Sanatorium). This will open a new door into The Tormented Asylum.
6. Here the first prison cell to your left will have webs all over it with a similar blue glow, Offer the Ravenous Medallion here which will give you the Decrepit Rune in return.

   

7. Take this back to Nimue’s Retreat and you will be able to craft the Rune Pistol with the help of Nimue.
